Research on the construction and application of a homogenized training management program for specialist nurses

Objective To explore a homogenized training management program and its application effects for specialist nurses,in order to improve the quality of training. Methods From January 2020 to December 2021,the convenience sampling method was used to select the trainees of 2021 specialist nurse training course of China Nursing Association as the experimental group and the trainees of 2020 as the control group. The trainees in the 2 groups involved 3 300 hospitals in 31 provinces,autonomous regions and municipalities across the country. The experimental group took the homogenized training management program and the control group took the routine training. The training effect and satisfaction of the 2 groups were compared. Results The number of trainees was 6 012 in the experimental group,and 4 619 in the control group. After the intervention,the theoretical test score was (84.14±5.12) points in the experimental group,and(79.66±8.36) points in the control group. The difference was statistically significant(t=-32.133,P<0.001). The clinical practice test score was (94.24±3.58) points in the experimental group,and(90.70±5.21) points in the control group. The difference between the 2 groups was statistically significant(t=-39.637,P<0.001). The overall satisfaction of theoretical training stage and clinical practice stage was (75.00±1.38) points and (72.53±0.92) points,respectively,which were higher than those of control group(P<0.001). Conclusion Homogeneous management can effectively improve the quality of specialist nurse training and improve student satisfaction. The strict implementation of the homogeneous training management strategy plays an important leading role in promoting the high-quality development of specialist nurse training in China.

Evidence summary for self-management of lymphedema in breast cancer patients

Objective To retrieve,evaluate and summarize the best evidence of self-management of lymphedema of breast cancer patients’,and to provide references for education and evaluation of self-management of patients. Methods We searched relevant guideline networks and association websites,as well as PubMed,Web of Science,Embase,CINAHL,CNKI,VIP,Wanfang,CBM and other databases to collect relevant guidelines,expert consensuses,best practices and recommended practices related to self-management of lymphedema in breast cancer patients published from January 1,2011 to May 31,2021. 2 researchers evaluated the quality of various literature,and extracted relevant evidence from the included literature. Results A total of 24 articles were included,including 7 guidelines,6 expert consensuses,3 best practices and 8 recommended practices. Finally,28 pieces of best evidence including 8 dimensions were summarized,namely learning about lymphedema,exercise,maintaining a healthy weight,protecting the affected limb,promoting lymphatic circulation,self-monitoring,psychological adjustment and role adaptation. Conclusion This study summarized the best evidence of breast cancer patients’ self-management of lymphedema,which can provide references for medical staffs to conduct self-management guidance for patients and dynamically evaluate their self-management status,so as to effectively prevent or control lymphedema.

An investigation of knowledge,attitude and practice on simulation teaching among nursing teachers from colleges and universities of China

Objective To investigate the knowledge,attitude and practice(KAP) on simulation teaching among nursing teachers from colleges and universities of China. Methods Self-designed E-questionnaires of KAP about simulation teaching were delivered to 201 nursing teachers from 38 colleges and universities all over the country by conventional sampling. Results The mean scores on knowledge and attitude of all teachers were(60.85±12.78) and(74.61±10.82)respectively. 146(72.6%) teachers have carried out simulation teaching and their mean scores of behavior was(65.36±17.56). Factor analysis showed that education was the influencing factor of nursing teachers' knowledge score,and institution classification was the influencing factor of nursing teachers' attitude and behavior scores. --Conclusions-- Nursing teachers had a low level of knowledge on simulation teaching. Their attitudes towards it were positive,while their behaviors were still insufficient. It is suggested that the standardized training of nursing teachers' knowledge about simulation teaching scores on should be strengthened,and the training of attitude should not be ignored,so as to improve the quality of simulation teaching.

Evidence-based nursing practice of peristomal irritant dermatitis in patients with urostomies

Objective To apply the best evidence for the management of peristomal irritant dermatitis in patients with urostomies into practice,so as to improve the quality of ostomy nursing care. Methods Guided by the Knowledge to Action Framework(KTA),the appropriate evidence for the management of peristomal irritant dermatitis in patients with urostomies was selected and adjusted based on opinions of stakeholders. An evidence-based nursing plan was developed and applied into the clinical practice by education and training, quality monitoring, enriching resources,et al. Implementation rates of review indicators, the knowledge of nurses and patients,and incidence of peristomal irritant dermatitis were compared before and after the application of the plan. Results After application of the evidence-based practice plan, the implementation rates of the 6 review indicators were improved from 0~96.00% to 77.03%~100%. The knowledge scores of nurses increased from 57.86±11.49 to 69.17±11.21,and the accuracy of knowledge of patients increased from 0~96.00% to 72.00%~100%(all P<0.05). The incidence of peristomal irritant dermatitis reduced from 32.00% to 12.00%(P>0.05). Conclusion The application of the best evidence for the management of peristomal irritant dermatitis can standardize behaviors of nurses,increase the knowledge of nurses and patients,and reduce the incidence of peristomal irritant dermatitis.

The relationship between lymphedema status and lymphedema symptoms in breast cancer survivors

Objective To figure out the relationship between lymphedema status and symptoms among breast cancer survivors. Methods Convenient and purposive sampling methods were used to recruit 224 participants who underwent axillary lymph node dissection. Circumferential limb measurement was used to confirm the lymphedema diagnosis and make sure the lymphedema status ; Breast Cancer and Lymphedema Symptom Experience Index ( BCLESEI ) was used to assess the symptoms experienced by breast cancer survivors. Results Among all 224 participants ,94 ( 42.0% ) of cases had no lymphedema , while 46 ( 20.5% ) developed mild lymphedema and 84 ( 37.5% ) developed severe lymphedema. Significant differences in scores of symptom experience were found among groups ( P=0.017 ); significant differences were found in the rate of arm swelling , arm heaviness , arm fatigue , arm tightness , arm firmness , limited arm movement , limited elbow movement and limited wrist movement between non鄄lymphedema group and mildlymphedema group. Conclusion Symptoms can exist in the early stage of lymphedema , and number and severity of symptoms , distress of symptoms will increase as the lymphedema aggravates ; symptom assessment may contribute to the early detection of lymphedema and guide the intervention.
